Specialist Referral & Coordination

Your referral begins with a conversation in which we review your history, current concerns, and any relevant labs or studies to identify the specialist best suited to your needs. We then send a focused summary directly to that physician, so they understand your situation before you arrive rather than starting from scratch.

Because we manage much of your chronic disease management and preventive care in-house, the specialist receives a complete, organized picture of your health. After your visit, their findings are folded back into your overall care plan so nothing falls through the cracks.

Before your appointment, gather a current list of your medications and supplements, along with any questions or symptoms you want to discuss. If you have recent imaging or outside records, let us know so we can include them in your referral.

We will confirm that any needed pre-visit testing, such as a lipid panel or EKG, is completed and forwarded ahead of time. This helps your specialist make the most of your visit and avoid duplicate testing.

Once you have seen the specialist, we review their recommendations together and integrate any new diagnoses, medications, or follow-up steps into your personalized care plan. If new prescriptions are involved, we perform a medication reconciliation to confirm everything works safely together.

We continue to monitor your progress, coordinate any additional visits, and remain your central point of contact, so you always have a physician who knows your full story.
